On the bottom of the card is an "issued date". This represents the date the card was issued. If it is a replacement it will show the replacement date and not the original issue date. The second change that you will see later this year. The last name or surname will be put on a separate line.

Identity Theft Using Your Social Security Number
Identity theft is one of the fastest growing crimes in America. The Social Security Administration gives some tips on home to keep your social security number protected.
- Remember you Social Security number is confidential. Be careful about sharing your number with anyone who asks for it.
- Don’t make it easy for someone to steal your number. Identity thieves get your personal information by stealing your purses, wallets, and mail. They can also get it by sorting through trash for personal data or even posing as someone who legitimately needs your information, such as a bank or employer.
- DO NOT carry your card with your. Show it to your employer when you start a new job, then put it in a safe place
If you suspect someone is your using your Social Security number, you need to file a complaint with the Federal Trade Commission by calling 1-877-IDTHEFT (1-877-438-4338) or online at www.consumer.gov/idtheft.
